Applying for Social Security Disability benefits means filling out a lot of forms. The form describing your work history – Form SSA-3369 – is one of the first ones you’ll see. It’s an important part of your disability claim.

This Work History Report is where you explain the jobs you held in the 15-year period before you experienced health problems that forced you from work. It will ask you describe the physical demands of each job.

WHAT INFORMATION DO YOU NEED FOR FORM SSA-3369?

You’ll need the following information for Form SSA-3369:

  • Job titles for each job
  • Dates you held each job
  • Types of tasks you performed
  • Supervisory roles you held
  • How much weight you lifted in each job
  • How long you had to stand in each job
  • How much you walked, climbed, stooped, kneeled or crawled in each job

WHERE DO YOU SEND FORM SSA-3369?

You can file Form SSA-3369 online with the Social Security Administration (SSA) at the same time you file for Social Security Disability benefits.

Or, if you apply in person, you can fill out your Work History Report during your interview with an SSA claims representative.

WHAT HAPPENS After You File FORM SSA-3369?

Along with Form SSA-3368, this report starts your Social Security Disability claim.

When you live in Louisiana or Mississippi, an SSA claims representative forwards this form to your state’s Disability Determination Services (DDS) office.

Each state has a DDS agency that makes an initial decision on your case for the SSA.

Then an SSA disability claims examiner evaluates your application.

If you’ve provided enough information, the examiner will get a clear idea of your situation.

And if your information is accurate, your claim will move more quickly through the system. That’s why it’s important to double-check points such as job titles and duties performed.

Missing some items or getting some information wrong could delay your benefits for months.
